The questions, section by section
Questionnaires differ in polish rather than substance. Nearly all of them cover five areas.
1. Height, weight and BMI
The gatekeeper question. UK providers prescribe Wegovy privately for adults with a BMI of 30 or above, or 27+ alongside a weight-related condition such as high blood pressure or type 2 diabetes. You enter height and current weight, and the form calculates BMI on the spot. Give your real numbers — most providers verify them against photos, and a prescription based on a made-up weight is a prescription based on nothing.
2. Your medical history
Expect direct questions about the conditions that change the risk picture: pancreatitis, gallbladder problems, thyroid disease, serious digestive conditions, diabetes and diabetic eye disease, and your mental-health history, including eating disorders. Some of these are outright stops; others just mean the prescriber wants more detail before deciding.
3. Medicines you already take
You'll list everything, but prescribers look hardest at diabetes medicines — insulin and sulfonylureas can cause low blood sugar alongside semaglutide — plus any other weight-loss medicine. Have your repeat-prescription list handy; it turns this section into thirty seconds' work.
4. Pregnancy and breastfeeding
Wegovy isn't prescribed during pregnancy or breastfeeding, and prescribers ask about your plans, not just your present. If you're trying to conceive, expect a no — and be glad the question was asked.
5. What you've tried so far
Most forms end with lifestyle questions: previous diets, activity levels, your goals. If the prescriber says no
Declines happen, and they're a feature of a safe system, not a failure of yours. The usual reasons: a BMI below the threshold, pregnancy or plans to conceive, a history of pancreatitis, medicines that don't mix with semaglutide, or answers the provider couldn't verify.
Ask the provider why — most will tell you — and if it's borderline, a chat with your own GP beats trying another questionnaire with shaded answers.
